This Safety Agreement is between Wei Wei Dumplings and the participant in cooking activities, or their parent(s)/legal guardian(s) if a participant is a minor. To help ensure the safety of all Wei Wei Dumplings students and employees, every person entering the studio must expressly agree and promise to abide by the safety precautions outlined below.
Each time a participant attend a class, they agree to and affirm the following:

Have/has not shown new or increased symptoms of COVID-19 in the past 10 days. According to the Center for the Disease Control, symptoms include:
If not fully vaccinated, have/has not been in contact with anyone who has tested positive for COVID-19 or shown any of the above symptoms in the past 10 days, including members of the participant’s household per the CDC guidelines.
If fully vaccinated, have/has not been in contact with anyone who has tested positive for COVID-19 or shown any of the above symptoms in the past 5 days and received a negative COVID test 3-5 days after exposure.
Agree to inform Commonwealth Circus Center if I learn that I have/my child was in direct contact with someone, within 10 days before coming to the studio, who later tested positive for COVID-19.
Have/has worn a protective mask when in public situations where social distancing is not consistently possible.
Understand that I could be a carrier of COVID-19 and be asymptomatic.
Understand that I could contract COVID-19 from an asymptomatic person or a contaminated surface at our facility.
